I haven’t heard of it being done before, but we don’t always get to know about the all customisations done.
Theoretically you could add a new question type that would be an upload field. You would definitely need to custom develop the core files which we obviously don’t recommend as those upgrades will need to be constantly maintained in order to keep up with updates.
You could add another plugins form to the event registration page, probably need some sort of ajax call to upload the item without the page refreshing though.
I am not sure how you would link it to that customer without them being registered with your site. If users are registered with your WordPress site and logged in, it would be easier, you could have a profile field on the front end, maybe using Gravity Forms or a custom solution, then the file/upload would be linked to that users profile, though not the attendees data.
So to answer your questions, easy ways? Probably not, but it is possible with some custom development.
