Is there a way to add an area or field for extra donations that is added to the total at the end?

Not currently. If you really need this feature we might consider adding it if you can help with the cost (more information about Sponsored Development sponsored development http://staging.eventespresso.com/about/philosophy/#sponsored-development).

The best thing to do is either add it to the ticket price or use the Multiple Event Registration addon (http://staging.eventespresso.com/product/espresso-multiple/) and let them add a donation amount to cart as a separate event.
